Note: Please mark Preferred your preferred choices, Back Up for your back up choices and Do Not Book for any that you absolutely do not want at your event.

Select 75% more topics than your space will allow to attend. The exhibitor topics are not guaranteed as some topics may not be available or may not pass our pre-screening process.

Note: Do Not advertise any specific exhibitor topic until HFD has confirmed that it has been booked and you have been notified they will be attending your event. Any medical exhibitors that are covered under your insurance plan(s) will include a minimum of 1 in-network doctor from each practice in their office. HFD will give you a list of these in-network doctor(s) that you can share with your employees. HFD strongly urges that you remind your employees to ask for the in-network doctor(s) whenever making an appointment at any medical practice. Please see our in-network policy on our website for full details.

Event Space Information

We need the following information so we can build a floor plan for your event:

1 - Send Over a hand sketch of the room noting the main details of the room, where we can fit tables and where the doors and electrical outlets are.

2 - You can upload pictures of the room indicating the room size, where the doors and electrical outlets are.

3 - Send over a copy of the buildings architectural drawings of the room.

In each case we need the following Information:

Event Order Confirmation
By signing below you authorize Health Fairs Direct to provide a health fair at your location according to the information listed above, unless amended in writing, three(3) weeks prior to your event. Additional services may be included such as medical screenings, via an additional order estimate. A formal contract/agreement is available upon request.
